When we get a bill that run a few pages long, we tend to check the main numbers and not the details unless there is a big change from month to month.
It happened to me with my Cell Phone bill until I noticed a suspicious charge last month for something we did not order.

Here is the official form you are supposed to fill out to complain about fradulent m-qube charges on your cell phone bill. Verisign owns M-qube.

I couldn't get ATT to remove the fradulent $9.99 monthly bill for m-qube on my daughter's bill until I asked to speak to a supervisor. Even threatening to cancel service didn't cause a reaction by the drone they have answer the phones. You can also get charges cancelled if you explain that the person who uses the phone is under age 13 and therefore legally cannot agree to a subscription charge.
This apparently is a negative opt in scam. That means they send you a text message, and if you don't respond to say you don't want it, you are subscribed. There is a special place in hell for these bastards.
