BP has announced the results of an internal investigation into the April 20, 2010 explosion and resulting fires of the Macondo Blowout on the BP Deepwater Horizon drilling rig, which led to the largest oil spill in history, and an accident which took 11 lives and injured 17 others.
In a report, BP blamed "a sequence of failures", including a "bad cement job" and misreading of pressure tests by engineers for the Gulf oil spill.
It identified eight factors behind the disaster, including mechanical failures, human judgements, engineering design and team interfaces.
The investigation is the first of a series of others, which will include US congressional and Department of Justice investigations.
The findings are not unexpected considering that BP must avoid being found guilty of negligence to avoid huge penalties.
