Evenflo High Chair Recall

Evenflo is recalling 643,000 high chairs after receiving hundreds of reports of seat backs coming loose or parts falling off, and injuring dozens of children.
Evenflo, based in Ohio, recalled 95,000 Majestic high chairs in December due to faulty plastic caps and metal screws falling off.
This recall, in cooperation with the US Consumer Product Safety Commission (CPSC), involves products sold across the US at stores including Toys 'R' Us, K-Mart and Burlington Coat Factory between December 2002 and April 2006.
Evenflo has received 320 reports of Envision seatbacks detaching or unexpectedly reclining, also a problem with the Majestic chairs. CPSC said there also have been 158 reports of fasteners, screws and plastic end caps falling off Majestic and Envision chairs. A total of 64 children have been reported injured, mostly by bumps and bruises to the head or other parts of the body.
The affected model numbers are:
2891321, 2891321A, 2891333, 2891351, 2891351A, 2891365, 2891375, 2891403, 2891403A, 2891466, 2891466A, 2891478, 2891536, 2891536A, 2891573, 2891586, 2892351 and 2892351A. The model number can be found on a white label on the seatback. "Evenflo" and "Envision" are printed on the front of the tray.
