The Garuda Indonesia Boeing 737 slammed hard onto the runway in Yogyakarta on March 7, careened off the end of the air strip into rice paddies and burst into flames.

Twenty-one people, including five Australians, were killed among the 133 passengers and seven crew on board.

The co-pilot and other parties were also at fault, the report by Indonesia's National Transport Safety Committee found.

It said the pilot disregarded 15 warnings from the Ground Proximity Warning System (GPWS) and landed even though the jet was going at "an excessive airspeed and steep flight path angle... resulting in an unstabilised approach."
