Dover Air Force Base admitted to cremating the remains of American troops and dumping them into a Virginia landfill. This practice went on for years.

Dover Air Force base Mortuary is the landing point for American war dead returned from overseas. The mortuary routinely mishandled remains, including removing body parts so that dead bodies could fit into uniforms for military funerals. Family members were not notified, and were in come cases directly lied to.
Nobody involved in this process has been fired over this.
This story was broken by the Washington Post, which was covering an investigation into Dover Air Force Base's mishandling of the remains of US troops.
The Air Force disciplined but did not fire the mortuary commander and two other senior officials. Some members of Congress called the punishments inadequate, and an independent federal watchdog agency said investigators should have pushed harder to assign blame.
